Clomiphene citrate (Clomid) is approved for female infertility, but it is also used off-label in selected men with low testosterone and fertility concerns. In men, it can stimulate the body’s own LH and FSH production, which may increase endogenous testosterone and support sperm production.
Clomiphene may be considered when a man has low testosterone and wants to preserve fertility. This is different from testosterone replacement therapy, which can suppress gonadotropins and sperm production.
TRT directly supplies testosterone. Clomiphene instead stimulates the hypothalamic-pituitary-testicular axis. The better option depends on the diagnosis, symptoms, fertility goals and laboratory findings.
Men with secondary hypogonadism may be more likely to respond than men with primary testicular failure. The underlying cause of low testosterone matters, so treatment should follow a proper evaluation rather than a single low lab value.
Monitoring may include total testosterone, free testosterone when appropriate, LH, FSH, estradiol and semen analysis when fertility is a goal. Symptoms and side effects are also reviewed over time.
Possible adverse effects include headache, mood changes, breast tenderness, visual symptoms and changes in estradiol. Any concerning symptoms should be reviewed with the prescribing clinician.
Weight loss is not an indication for clomiphene and should not be used as evidence that treatment is working. Response should be judged by symptoms, hormone levels and fertility outcomes when relevant.
