Phentermine is a sympathomimetic appetite suppressant used in weight management. Patients with thyroid disease often ask whether it changes TSH, T4 or T3 results. Phentermine is not a thyroid hormone and is not used to treat hypothyroidism.

Does Phentermine Directly Change Thyroid Tests?
There is limited evidence that therapeutic phentermine causes predictable, clinically meaningful changes in thyroid laboratory values. If thyroid tests are abnormal, clinicians should evaluate the result on its own merits rather than assuming phentermine is the cause.
Why Thyroid History Still Matters
Phentermine can increase heart rate, blood pressure, restlessness or tremor. Those effects can overlap with symptoms of hyperthyroidism. For that reason, active hyperthyroidism is an important contraindication to sympathomimetic weight-loss therapy.
What About Hypothyroidism?
Patients with adequately treated hypothyroidism may still be candidates for weight-management treatment depending on the rest of their medical history. Thyroid replacement doses should be adjusted according to thyroid testing and clinical need, not to accelerate weight loss.
Should You Stop Phentermine Before a Thyroid Test?
Do not stop a prescribed medication solely for laboratory testing unless the prescribing clinician advises it. Instead, tell the clinician or laboratory which medications and supplements you are taking so results can be interpreted appropriately.
When to Recheck Thyroid Function
Testing may be appropriate when symptoms suggest thyroid disease, after thyroid medication changes, or as part of evaluation for unexplained weight change. TSH is generally the first-line test for primary thyroid dysfunction, with additional testing as indicated.
